General description: Metabolic preparation with antiarrhythmic action.

The main properties of the dosage form: Capsules of 0.36 and injection form, 10% solution in ampoules of 5 ml and 10 ml.

Active substance:

2,3,4,5,6-pentahydroxy-caproic (gluconic) acid in the form of magnesium and potassium salts.

Form of issue. Capsules and solution in ampoules for injection.

Pharmacological properties.

RITMOKOR is the original combined preparation possessing metabolic, membrane stabilizing, antioxidant and antiarrhythmic action.

The expressed metabolic activity of RITMOCOR is due to the activation of redox enzymes, an increase in the intracellular level of ATP and creatine phosphate, which optimizes the functioning of ionic cell pumps. One of the mechanisms of the action of RITMOCOR on cells under conditions of ischemia and hypoxia is the inhibition of the intensity of free radical oxidation of proteins, lipid peroxidation. The use of the drug in conditions associated with the activation of free radical reactions is accompanied by a decrease in the permeability of cell membranes.

The drug potentiates the clinical effectiveness of antiarrhythmic drugs. Under the experimental conditions, it exhibits a tranquilizing effect.

Pharmacokinetics.

Quickly adsorbed from the gastrointestinal tract. The maximum concentration in the blood is determined after 1-1,5 hours. Within 48 hours, 95% of the drug is metabolized. Metabolism products are excreted by the kidneys.

Indications for use.

The preparation RITMOKOR is shown in complex treatment:
ischemic heart disease;
neurocirculatory dystonia;
myocardial infarction (for the prevention and treatment of heart rhythm disturbances);
disorders of the heart rhythm (atrial and ventricular extrasystolic arrhythmia)
inflammatory diseases of the heart muscle (myocarditis);
myocardiopathy;
disturbances of electrolyte exchange for correction of magnesium and potassium ions level;
liver diseases of various etiologies, for increasing the resistance of hepatocytes to various pathological influences (viral, inflammatory, intoxication);
chronic fatigue syndrome;

As an antiarrhythmic drug, RITMOCOR is used in the following cases:

1. Orally:
extrasystolic arrhythmia;
heart rhythm disturbances in hypokalemia and hypomagnesemia;
disturbances of the rhythm of the heart against the background of an overdose of cardiac glycosides;
prevention of paroxysmal atrial fibrillation, regardless of the frequency of ventricular contraction;

2. Parenteral:
pirouette-tachycardia;
rhythm disturbances in the presence of an overdose of cardiac glycosides;
disorders (prevention and treatment) of rhythm in acute myocardial infarction;
atrial and ventricular extrasystolic arrhythmia;
paroxysmal form of atrial fibrillation, regardless of the frequency of ventricular contraction;
reciprocal nodal tachycardia;

Method of administration and dose. In each case, the method of application of the drug is determined by the doctor.

Capsules take inside, 1-2 capsules for 10-15 minutes before meals, 3-4 times a day. In more severe cases, you can take 3 capsules 3 times a day. After 5-7 days the dose is reduced to one capsule 3 times a day. The duration of the course of taking the drug is determined individually.

Parenteral RITMOCOR is administered intravenously in a stream or drip.

For arresting an attack of arrhythmia, 5-10 ml of RITMOCORA solution is diluted in 10-20 ml of isotonic sodium chloride solution or 5% glucose solution and used as an intravenous infusion (within 3-5 minutes) or intravenously drip (30-60 drops per minute) , previously diluting the drug in 250-500 ml of isotonic sodium chloride solution, or in a 5% solution of glucose.

Children aged 1 to 3 years are injected with intravenous infusion of 1-2 ml of RITMOCOR for 1 year of life; at the age of 3-6 years 1 capsule 2-3 times a day or 5 ml of solution, 6 12 years 1 capsule 3 times a day or 5 10 ml of solution.Children older than 12 years, the drug is prescribed as an adult. In some cases, the dose is set by the doctor individually.

Side effect. The drug is well tolerated, side effects are extremely rare. When using the drug in large doses, dizziness, drowsiness is possible. In these cases, you need to reduce the dose of the drug. After stopping the side effects quickly pass.

Contraindications. Atrioventricular block of II-III degree, coma of unknown etiology, decompensated forms of diabetes mellitus, acute and chronic renal failure of II-IV degree. RHYTHMOCOR is contraindicated in hyperkalemia and hypermagnesia, as well as with increased sensitivity to the drug or its components.

Features of application. When appointing RITMOKOR patients with diabetes should take into account the possibility of determining a false-elevated blood glucose level in the surveyed because of the presence of gluconic acid in the glucose derivative of glucose.

One capsule contains 0.03 bread units, 1 ml of 10% solution contains 0.008 bread units.

Interaction with other drugs. When RITMOCOR is used simultaneously with antiarrhythmic and antihypertensive agents, their mutual enhancement of activity should be taken into account. With the simultaneous use of the drug with potassium-sparing diuretics and angiotensin-converting enzyme inhibitors, control of potassium and magnesium levels in blood serum is necessary. With simultaneous use with cardiac glycosides, the risk of their toxic effects on the heart decreases.

Conditions and shelf life. Store in a dry, protected from light and out of reach of children at a temperature of +15 to +25 oC. Shelf life 3 years.
